You need that form or else Immigration won't have proof of employment.
Looks like your company does not want to pay their share of your CPF.

ignore the above poster.  you can submit without it, just include photocopies of payslips, your tax assessment notice etc etc

why rush applying for pr you are only working in sg for 4 months. and it says there i you are s pass holder you need to atleast been working in singapore for 2 years or more. try to look up at MOM website you can see what i'm telling. Annex A is needed also otherwise immigration won't accept it.

Annex A is not compulsory for PR application. I got my PR approved without it.

is there any possibility that PR will be approved under the following condition:

** SPASS ; with 1 yr and 6 months work experience ; salary range of 2.1 to 2.3 K.

Please advise.Thanks!

Normally, the guru advised SP holder is better to apply after 2 years.

However, if you want to apply it due to some reason now, then just do it, there are no harm, however, considering current resession, it might take long time to proceed the application, it might take 3-6 months, three of my colleagues with EP P2 holder are at this processing, it is already 3 months passed since application, however, they got it it is still under processing when they called ICA for status...
